BAME HEALTHCARE SURVEY

WHY HAVE I BEEN INVITED?
You have been invited to complete the survey because you are an adult member of a BAME group living with HIV in the UK. We invite you to share your unique experiences and contribute to the knowledge around how stigma and discrimination affect the lives of BAME people living with HIV within the healthcare setting.
HOW DO I TAKE PART?
The online survey will take about 15 minutes to complete. You can do this on any device with internet access (phone, tablet or computer). Once you begin, you are not able to log out of the survey and come back later to complete it so you need to complete all questions in one session.
For the research to be most effective, please answer all of the questions even if they may be sensitive in nature.
WHY SHOULD I TAKE PART?
Data from this survey will help us better understand experiences within healthcare for HIV-positive service users. This has significant implications for standard of care and could lead to improvements that ensure people living with HIV are not treated differently to others in this setting or avoiding care for reasons related to the way their treatment is provided.
At the end of the survey, you will be invited to enter a prize draw to win an iPad. To enter, you must provide an email address or phone number for us to contact you if you win. Contact details will be used for the prize draw ONLY, and will not be linked to your survey responses. Winners will be randomly selected from eligible entries at the end of the survey period.
The number of entries eligible for each draw will be published at www.stigmaindexuk.org and winners will be informed by 1st May 2017. Multiple entries will be disqualified.
